We are recruiting three (two – 1.0 FTE, one – 0.9 FTE) oncology/hematology infusion center pharmacists to join our team. The position primarily works day shifts, Monday through Friday, with limited rotating weekends and no scheduled holidays. Hours and weekend coverage may evolve based on future Cancer Center needs.Position description: The oncology/hematology infusion pharmacist is responsible for drug distribution, drug therapy monitoring, patient education, chemotherapy order processing, product verification, precepting learners/residents, participating in research projects, leading and participating in quality improvement initiatives, and providing integrated care and medication therapy management services to patients receiving treatment. This individual will also conduct comprehensive patient clinical reviews, assist with clinical consults, and support the medication use process for patients on investigational therapies/cancer clinical trials. This individual will develop and maintain strong relationships with infusion nursing and clinic staff, medical oncology staff, and other members of the care team. This position will be based in Rochester.Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.Preferred: Completion of an accredited pharmacy residency. Experience working in a large, progressive academic medical center. Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred. Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred. Experience and/or interest in teaching and/or precepting preferred. Licensure: Current pharmacist license by the Board of Pharmacy in Minnesota.
